Position Summary

As the Director of Virtual Care Management, you will be a clinical and operational lead for PreemptiveClinic’s fully virtual care platform. You’ll collaborate closely with our MD medical directors and leadership team to develop protocols, build and inspire a high-performing clinical workforce, and continuously optimize our RPM and care navigation services. A strong entrepreneurial mindset, coupled with a passion for transforming patient outcomes, is essential.

Key Responsibilities

Strategic Leadership & Team Building: Recruit, mentor, and lead a diverse remote care management team. Develop and implement workflows, performance metrics, and best practices for virtual clinical services.

Clinical Oversight: Ensure high-quality care delivery, regulatory compliance, and alignment with evidence-based practices specific to virtual remote patient monitoring and care navigation.

Operational Excellence: Streamline and standardize care processes, from patient enrollment to discharge planning, to maximize efficiency and patient satisfaction.

Collaboration & Communication: Serve as the primary liaison among clinical staff, medical directors, and technology teams; foster cross-functional cooperation to enhance patient-centered care.

Continuous Improvement: Identify opportunities for innovation and performance optimization in telehealth delivery, leveraging data analytics and user feedback.

Qualifications

Education & Licensure: Bachelor’s degree in Nursing (Master’s preferred). Current PA (or RN) license in good standing.

Experience: Minimum three years of progressive leadership experience in care/case management (five years preferred), ideally with exposure to remote monitoring or telehealth services.

Technical Proficiency: Familiarity with RPM platforms, electronic health records, and data analytics to evaluate quality and cost-effectiveness.

Entrepreneurial Mindset: Demonstrated ability to innovate, adapt, and implement solutions in a fast-paced, technology-driven environment.
