The Opportunity

We are seeking a Licensed Practical Nurse (LPN) to serve as the face of Preemptive Health in the Ocala area. In this role, you’ll be embedded directly in the community—working onsite at senior living facilities, 55+ communities, or churches.

This is not just a clinical position—it’s a chance to lead from the front. You’ll provide trusted care, support new member enrollment, and strengthen the bond between our members, their families, and the Preemptive team. What You’ll Do

  • Serve as the primary Preemptive Health presence in your assigned facility or community.
  • Recruit and enroll new members by explaining our preventive care model and earning their trust.
  • Verify demographics and insurance, obtain informed consent, and document basic health history.
  • Capture baseline vitals (blood pressure, pulse oximetry, weight) and perform a smartphone blood-flow scan using our proprietary app.
  • Provide ongoing health education, device support, and check-ins with members.
  • Identify and escalate situations according to clinical protocol.
  • Document interactions in our EMR/CRM and coordinate follow-up with NPs, RNs, and physicians.
  • Partner with community leaders to build awareness of Preemptive Health and strengthen relationships.

Must-Have Qualifications

  • Active, unrestricted LPN license (Florida or multistate compact).
  • 1+ year of experience in ambulatory, community health, home health, or primary-care settings.
  • Strong communication skills; comfortable explaining care models and building trust with diverse senior populations.
  • Tech-savvy—confident with tablets, Bluetooth medical devices, and cloud-based EMRs.

Nice-to-Have

  • Experience with remote patient monitoring (RPM), chronic care management, or value-based care.
  • Spanish or other community language skills.
