The Role

As a Nurse Practitioner on our Virtual Care Management team, you’ll deliver an exceptional, member-first clinical experience. You’ll report to the Director of Virtual Care Management and partner closely with our physician medical directors, care managers, and care coordinators.

This role blends:

✅ Virtual clinical care (onboarding + episodic support)

✅ RPM review + triage

✅ Care plan collaboration

✅ Day-to-day clinical leadership for the care team

✅ Process improvement as we scale

What You’ll Do

🩺 Clinical Care

  • Conduct virtual onboarding E/M visits (history, assessment, documentation, orders/referrals, follow-up plans)
  • Provide clinical decision-making between visits; ensure timely care and appropriate escalation
  • Coordinate with member PCPs and specialists to support continuity of care

📈 RPM Review & Triage

  • Review RPM data, alerts, and trends to identify members needing outreach or escalation
  • Document clinical interpretation and actions taken; route follow-ups to the right team member
  • Escalate clinical concerns to the Director and/or medical directors per protocol

🤝 Care Planning & Collaboration

  • Partner with care managers to design/review/approve individualized care plans aligned to evidence-based guidance + member goals
  • Support prioritization, next steps, and changes in member status
  • Ensure documentation supports consistent, high-quality care delivery

👩 ⚕️ Care Team Leadership

  • Provide day-to-day clinical guidance to care coordinators and care managers; unblock escalations
  • Mentor on member communication, documentation quality, and escalation judgment
  • Promote a calm, empathetic, member-first approach across the team

🔧 Program Development & Process Improvement

  • Help refine clinical workflows, templates, and protocols to improve experience, safety, and efficiency
  • Identify gaps and propose improvements based on recurring needs, outcomes, and team feedback
  • Contribute to building and scaling a modern care management program in an early-stage environment

What We’re Looking For

  • Active, unrestricted Florida NP license in good standing (required)
  • 2+ years NP experience providing direct patient care (virtual care experience is a plus)
  • Strong E/M fundamentals: assessment, treatment planning, documentation, ordering/referrals
  • Comfort managing chronic and/or complex conditions; knows when higher-acuity evaluation is needed
  • Experience with telehealth, RPM, care management, or care navigation preferred
  • Excellent communication: empathy + clarity + ability to drive follow-through
  • Strong collaboration skills with care teams; good escalation judgment
  • High attention to detail with documentation and follow-up across multiple members
  • Comfortable in EHRs and digital tools; learns systems quickly
  • Leadership/mentorship experience with clinical teams is a plus
  • “Builder” mindset: excited to improve workflows and playbooks as we grow
  • Ownership mentality and comfort operating in a startup where priorities evolve
