🎯 The Opportunity
We’re looking for a driven Care Navigator who thrives at the intersection of empathy, communication, and execution. You’ll support Medicare-age patients by helping them follow their care plan, overcome barriers (including social needs), and stay engaged with their care team and program. This is a high-impact, fast-paced role at the front line of a growing healthcare technology company. It’s a strong fit for someone who loves building trust, communicating clearly, and helping people take action that improves their health.
💼 What You’ll Do
- Build trusted relationships with members through structured check-ins that clarify goals, priorities, and what’s getting in the way, then turn that into a simple, practical next-step plan.
- Help members navigate what to do next in their care by coordinating appointments, referrals, follow-ups, and services.
- Identify real-world barriers that impact health (transportation, food access, housing stability, utilities, caregiving support, benefits) and connect members to appropriate resources.
- Support members after key moments like clinician visits, ER visits, or hospital discharge by reinforcing instructions, confirming medications and equipment, scheduling timely follow-ups, and escalating clinical concerns.
- Maintain high-quality documentation in the clinical platform: action plans, outreach attempts, referrals, and follow-ups, with strong accuracy and follow-through.

- You’re organized in a way that shows up in your work: clean notes, reliable follow-ups, accurate tracking, and no dropped handoffs.
- You communicate clearly across phone, text, and email, and you can adapt your style to the person in front of you.
📘 Qualifications
- Bachelor’s degree (or equivalent practical experience)
- Strong empathy and people skills, with the ability to build trust quickly with Medicare-age adults and support them through stressful or confusing situations
- Excellent communication across phone, text, and email, including clear step-by-step guidance and respectful persistence with follow-ups
- Comfortable identifying real-world barriers that affect health (transportation, food access, housing stability, utilities, caregiving support, benefits) and helping members access appropriate resources
- Tech savvy and fast-learning across tools like CRM, calendars, dashboards, and spreadsheets
- Highly organized with strong execution: clean documentation, reliable task tracking, closed-loop follow-through, and tight handoffs across teams
- Good judgment and professionalism: knows when to escalate concerns to the RN/clinical team, protects privacy, and communicates accurately under time pressure

💰 Compensation and Logistics
- Contract - 1099
- Salary: $27.50 - 35 / hr
- Fully Remote
