The Opportunity
We're looking for a Registered Nurse who brings strong clinical judgment, calm communication, and a genuine passion for caring for their patients. As a Nurse Care Manager, you'll support Medicare-age members virtually through proactive check-ins, remote patient monitoring (RPM), chronic care management (CCM), review, triage, care coordination, and education. You'll work alongside our Nurse Practitioners, physician leaders, and care team to deliver a concierge-level experience that helps patients stay healthy, independent, and ahead of problems. This is a fully remote position with consistent weekday hours (Monday–Friday, ET). It's a high-impact role at the front line of a growing healthcare technology company, and a strong fit for someone who values relationship-driven clinical work over high-acuity chaos.
What You'll Do
Conduct virtual visits with members by phone and video to assess symptoms, surface concerns, reinforce care plans, and provide coaching on chronic conditions and prevention-focused goals.
Review incoming physiological data, identify meaningful trends, and triage member concerns using clinical protocols, escalating to NP or physician when appropriate.
Coordinate follow-ups, referrals, and next steps across the clinical team after key moments like clinician visits, ER visits, or hospital discharges, reinforcing instructions and closing loops with members and families.
Maintain high-quality documentation in the clinical platform: assessments, interventions, outreach attempts, triage decisions, and follow-ups, with strong accuracy and follow-through.
Contribute to protocol refinement, member messaging, and workflow improvements as we scale the care model.

You're confident triaging and escalating using protocols, and comfortable making sound judgment calls in ambiguous situations.
You're organized in a way that shows up in your work: clean documentation, reliable follow-ups, accurate tracking, and no dropped handoffs.
You're self-directed and dependable in a fully remote environment, and you bring a steady, reassuring presence to every member interaction.
Qualifications
Active compact nurse license (RN or LPN) license in good standing
3+ years of nursing experience (care management, primary care, geriatrics, ambulatory, or telehealth a plus)
Strong phone and video communication skills with high emotional intelligence, particularly with Medicare-age adults
Tech savvy and fast-learning across tools like EMRs, chat platforms, video calls, dashboards, and spreadsheets
Confident triaging and escalating appropriately using clinical protocols
Good judgment and professionalism: knows when to loop in NPs and physician leaders, protects privacy, and communicates accurately under time pressure
Growth mindset
Nice to Have
Experience with RPM or CCM programs or value-based care
Background in geriatrics, home health, community health, or preventive care
Spanish or other community language skills
Prior startup or high-growth environment experience

Salary: $58,000–$72,000/year, based on experience
